Pens\u00e9 comme un v\u00e9ritable outil au service des personnes accompagn\u00e9es, il renforce la qualit\u00e9, la continuit\u00e9 et la lisibilit\u00e9 des parcours.<\/p><p>Concr\u00e8tement, cette d\u00e9marche permet :<\/p><ul><li>Une s\u00e9curisation et une meilleure circulation de l\u2019information<\/li><li>Une coordination renforc\u00e9e entre les \u00e9quipes et les partenaires<\/li><li>Des projets personnalis\u00e9s plus partag\u00e9s et coconstruits en coh\u00e9rence avec nos valeurs d\u2019autod\u00e9termination et d\u2019inclusion<\/li><\/ul><p>Cette transformation num\u00e9rique est avant tout une aventure collective. Elle mobilise les professionnels, soutient l\u2019innovation dans les organisations et contribue \u00e0 faire \u00e9voluer durablement les pratiques.<\/p><p>Merci \u00e0 l\u2019ensemble des
